Rose City is eager to invite new carriers into our workspace.  In the effort to become a “qualified” service provider for our Company, the following information must be submitted…

CARRIER REQUIREMENTS:

•             Legal Business Name

•             Complete Mailing and/or Physical Address

•             Company Phone, E;mail, Fax, & Mobile Phone Numbers

•             Completed IRS Form W-9

•             FMCSA Operating Authority – DOT & MC Numbers

•             Certificate of Insurance - Must List Rose City Transportation, Inc. as the “Certificate Holder” with the following Minimum “All Risk” and “Broad Form” Coverage Limits:

   Automobile Liability Coverage:               $1,000,000

   Cargo Coverage:                                              $100,000

•             Reefer Breakdown Coverage must also be validated, and noted on your “Certificate”, to transport Temperature Controlled and/or perishable freight.          

All Certificates of Insurance must be named Rose City Transportation, Inc. as the Certificate Holder

•             Signed Rose City Transportation, Inc. “Broker-Carrier” Motor Transportation Contract
